Dan appeared four times on Johnny Carson's "Tonight Show" and this video contains all the footage available from his first and most famous appearance back in April of 1968.  It is a mix of the interview between Johnny and Dan (in black and white) and the actual pitching demonstration in color (as seen below in still clips)...
